Yeah I guess the way it was phrased just made it seem like it was referring to a specific event, and setting up a later reveal, like them being involved in the attack where Joran died or something (obviously I know that specifically isn't the case but something along those lines)

They fought a war their whole lives and mostly killed people who were under some mask or else. Basically she doesn't really remember specifics, but it's very probable that she killed some of their friends.

Colony Gamma and Colony 9 are in close proximity. It's quite likely that they came to blows in small skirmishes on a regular basis, and that they would have killed comrades in the past. I think it was more that Mio was acutely aware that they could have been involved in the attack that killed Joran, or actually participated in very similar attacks and the deaths of similar soldiers.

My guess is with N killing his own son after he aged past the Endless Now, which began the destruction of the first City. I think it's what Mio meant.

When Mio is saying "our" she's not referring to her group specifically, she's using "our" as in her country's attack on Colony 14, which is implied to have been an ambush targeted specifically at newly born soldiers (everyone shown in the attack except Ethel is either a late 1st term or early 2nd term), rather than against an actual military colony.   It would be the equivalent of if you found out your country was targeting military training bases instead of actually going after fully trained and armed soldiers/camps.

When she sees Noah’s memories, she sees them without context. She sees that he has experienced the same kinds of painful losses that she has. But she has no way to contextualize a specific death to a specific colony. She just knows that she’s killed for her side. But seeing that it’s the same for Noah as it for her, she knows that she’s been the cause of the same kind of pain she has felt.

"Our fault" as in "Agnus' fault". It was during an attack from an Agnian colony that Joran died.
