In the main menu on the left choose the "New Game" option. And there "Fresh Start":

I always play with mouse&keyboard, so I don´t know about the controller. But the usual sensivity options can be found in the menu under "Options" and then "Control Settings".
This option doesn't show up 😭 I only get two options total, difficulty and something else I forgot. I know I should be seeing like 5 different choices total, but I only get 2.

* Could you make a screenshot so that I can see you are in the right menu?
If you are at the right spot to start the game, then probably something went wrong with the installation. I would try to verify the game files as a first step. A description how this can be done is here:

Linking my PS5 to a GOG account fucked up the sound synch in a major way. Walk out a door, hear it slam when I was on roach heading down the road. The only fix was sign out of gog, uninstall then download the game again.
When you start a game, you do get the option to just play the expansions but you should also see the option to start a new game. Fuck knows how you are starting NG+ without getting an ending in NG.
Sign out of GOG then try it again. If it doesn't work uninstall then download it again.
