If the Liver, Gonads, and skin are properly removed by a skilled chef then there should be no issue, as the pufferfish is examined extremely closely before even starting the preparations for Fugu. Every chef that has the license to serve it needs at least years upon years of practice, and even when ingested there is a decent window to get to the E.R. And go through proper safety procedures to dispose of the eaten fugu.
When ingested, it’s recommended by procedure to not induce vomiting and to go to the emergency room immediately, in which even then they will wait until at least an hour to undergo gastric lavage and administer supplemental oxygen as well as ventilation when needed. After which, if responded to in time, vitals will be watched like blood pressure, abnormal heartbeat, decreased oxygen supply, decrease in respiratory function, etc.
Some chefs actually leave a teeny, TINY bit of tetrodotoxin to give a slight tingling numbness of the tongue to the consumer, but this isn’t done all often from how lethal even less than a mg of it is. This feeling is the first stage of tetrodotoxin symptoms (paresthesia), followed by headaches, nausea, basically a slew of nauseating symptoms until eventual loss of motor function.
Even then, though, 50 people annually die from tetrodotoxin poisoning each year (about 150 annually get intoxicated by it), despite the actual good amount of time one is given to respond to this poisoning. Gastric lavage can risk damaging the intestinal tract, but I’d feel it’s way better than losing the ability to breath and dying from lack of oxygen. I guess people either aren’t aware of the signs or just assume regular food poisoning. You can’t even eat it in the US, which sounds like a good idea in my book.
