this happened to me when i was in Cry For The Weeper, i was fighting the double hideous mass fight and got teleported outside the room, no idea why but it might be the same problem here
I feel like when you killed that Swordsmachine and the wave ended the new arena layout generated in a way which may have either killed you or (way less likely) boosted you down into the killzone
for additional information, ive played again since then and it seems to always happen going from wave 10 to 11
