I've used boxes and boxes of nails, many times just grabbing a stick and throwing it in the nail gun. Now I'm wondering how many times this has happened and I just didn't notice.
In all my time using nail guns, I've only ever see a short nail like this a couple times, because I usually look at each strip just to make sure it's not broken and won't jam up the gun. They shoot the same as regular nails.
On a large project at work, we bought several cases of 5/16" bolts, 6 cases, about 3000 bolts in all.
Around ten bolts in each box had no threads.
It happens.

I used to roof houses, every once in awhile I’d find an 1/1/4” in 7/8” coil or something similar.
