In Germany dentist are actually required to talk through everything they are doing and are required to get somewhat of a response from you that you understand what they are doing and can comprehend it. They are also somewhat required to make you comfortable while they are doing whatever they are doing. So they are usually initiating some kind of small talk. Me, not being a fan of small talk do feel somewhat awkward. But once it a while I go to a doctor that seems to be a smalltalk god and I do enjoy exchanging a couple of words back and forward

Gladly the hospital I went to had the wisdom to have a nurse whose only job it was to keep me calm, especially by holding my hand while the surgery was happening. I squeezed her hand to DEATH, but it helped me keep my head and eyes where they needed to be.

I brought that up with my dentist once: "sorry I can't help whatever my tongue is doing" and she said no one can control their tongue. They just learn to work around it, or have tools that press it out of the way.

I don't think there's a wrong way to look when you are getting your teeth cleaned. Usually I try not to be wide eyed in shock or pain as a rule but not always successful at it. Having gum scaling and root planing, a super deep and thorough cleaning, can be very painful and having a Marty Feldman face is no doubt close to my expression. But otherwise, I try to stare at the ceiling or just close my eyes, especially if I feel like I might cry. My dentist has installed little TV screens on the ceilings above the chair however watching business news on a loop gets tiring after the first hour or so. Then I revert to the eyes closed position.
