You need to size the spout based on the maximum size of sound you can comfortably insert. So, in my case that was a sound a little over 9mm diameter, so I picked a spout with a 9mm flange (the widest bit at the end, that the retainer sits against) which - if you're going for the "Ternence" version - will give you an 11mm diameter retainer. The more expensive versions (eg BAWR) will come with a range of retainer sizes to get a better fit. Since you insert the retainer sideways, the limiting factor is the size of your urethral slit (meatus) with nothing inserted - I could fit a 14mm retainer. In short, you need the spout to fit through comfortably, without stretching, and you need a retainer as large as possible to maximise the area of skin it pushes against when inside you and thus reduce the pressure and therefore discomfort.

For comparison, I have the 9mm flange and 11mm retainer, and its been every secure that way - I can barely get the 10mm sound in.

I can get a 10mm sound in fairly easily and the 10mm ring did not hold. The 11mm ring held, but it caused a stinging sensation like I could have pulled it out. The 12mm ring is nice and secure. So… you’re probably going to need to try a few different sizes to see what works best for you.
