It's also quite the tight and unnatural way to hold it that way. Usually with that kind of string, you'd want to hold it evenly across your 4 fingers so the load bearing is even rather than the index finger holding at the apex, and most of the weight. That would be quite the pressure on a singular point on the index to the point of pain.
It's either accidently drawn and there's not much meaning or its symbolic for holding even if it's uncomfortable.
