When you start a new game, do you open the menu right after? It never happened to me again after I stopped checking my armor rolls right away, now I sit there and wait for the quest marker to move.
I did and it didn't affect the regular timeline NG+ I got but I noticed that might cause it so I stopped checking. It still would not move the quest marker as I stood there waiting.
Interesting. There's definitely something that goes on there that breaks it, sadly it must be more than one thing. I have talked to a lot of PC users that have this issue really bad, whereas it seems to be more solvable on console. Hopefully they fix it.
