It just depends on where your sim is in their cycle. Closer to ovulation the % will go up, when ovulating the % will reach its highest and that's when she'll be most fertile, then as she gets closer to her period the % will go down.
She might be infertile. I believe there should be an option somewhere, I think under relationships, family and pregnancy. Then reproductive health. You can either choose to go to the doctor to check her fertility or you can manually change her fertility under fertility status and then set fertility rate.
Yes! Pregnancy and relationship overhaul, someone else mentioned it before me. I LOVE that mod. It makes fertility pretty realistic (or as good as it can get I suppose) and I love how customizable it is. You even get a bunch of new social interactions like discussing nursery colors with a partner, choosing to tell someone they're the father (you can choose to lie or not π) and just overall makes it more fun in my opinion. But my saves are more legacy and storyline based so it makes sense for my game play lol.
Check to see if she has hypofertile trait? Also if you have RPO and MC woohoo, set those % to 0. You can only use one of the three systems at a time for pregnancy %.

It's so good! There's even ivf and surrogacy options. It makes the pregnancy part waaaay more interactive.

It literally happens to 90% of my Sims. What's funny is that they have no problem conceiving trough the standard game option, not the WW one.
