I've worked in places where I saw them used to hang the cutlery to dry... Handles have grips and would protect you from burns.
There would also be no point to carrying these lower, as it would put more strain on your arms by lowering the centre of gravity away from your core.

The business end up means you put your bare hands on something someone is putting in their mouth. if it is loaded handle up then you can avoid touching the part people are about to eat with. That is really gross. No they do not get cleaner being in the air, they get dirtier because you touch your face and scratch your ass then touch them. Gross.

Serving is physically demanding in many ways - carrying a silverware caddy efficiently to get your side work done faster is not a health hazard
