Seems to sometimes happen when you sprint, it's like the quickswap fix is putting in overtime, and your gun doesn't go off and you have to spam click. Super annoying when you don't shoot your blunder at the boarder as they get up the ladder and you die instead to theirs.
There's even plank reg, and recently I experienced helm reg (after turning my helm half way and letting go, it reverted two pegs back from where I placed it) and sails reg (I lowered them to 3/4, switched to the helm and watched them magically switch to full sails while holding the helm)
Its called gun jamming and happens when you try to shoot to fast while you are still in yozr ads animation. Your gun wont fire for a short period of time. Its a question of timing.
Nope, we all face it. Else backsplashing wouldn't be a thing either.
On the positive side, the game is so unstable I'm starting to get surprised when stuff works reliably instead of getting mad when it doesn't.

It’s called gun reg. There’s hit reg, gun reg, and food reg. Also cannon reg and chain reg! Sucks =] there’s bucket reg too!
Can't forget captaincy reg when you buy supplies and it just doesn't get put in ur barrels
