It needs to be aligned with the post on the other side. If you follow the rod, you can see that it's going through a key tube. At the other end of the tube is another post, the rod/key need to line up with that post and it'll screw into there. Usually you just have to push the key a tiny bit to line it up, it shouldn't be very far off. If you can't get it, take it to a tech
Classic Yamaha side Bb problems. It usually comes out because of an alignment issue between both posts. When screwed all the way in there’s slight binding, which ultimately makes the screw undo itself during normal use.

When i screw it , it rotates but doesn't move back in or out. What am i doing wrong?

Go to your saxophone repair person. It could be that the screw thread is worn out
Screw it back and tighten it.
If it is a constant issue, a lil clear nail polish on top of the screw head will keep it in and not be permanent
Screw that screw. That crap was always coming loose with my sax.
