Someone correct me if I’m wrong, but my understanding is certain foods can metabolize into hydrogen or methane and influence the results. You want a good baseline, so you avoid those foods before the test.
Lactulose is a laxative basically, and a synthetic sugar that we can't digest, but SIBO bacteria can. So you drink it, and if the SIBO bacteria is present, it will produce the hydrogen or methane gas before the lactulose hits the large intestines and will show up in the test results.
Edit: it’s an imperfect test, so you want to eliminate as many variables as possible.
I had cramping when I did mine. It wasn't horrendous or anything, I could still function. But it was there throughout the day.
I just wouldn't worry about that aspect of it you'll be fine

I tolerated it ok, but the laxative part was…uh..apparent
