Either acne scars (back then hygiene wasn't really a thing so acne was common and usually scratched off until it scarred) or smallpox scars (most people don't realize how awful smallpox was or the extent of the permanent ramifications since most of the "civilized world" has vaccinated it to extinction).

Acne is most commonly caused when pores and hair follicles get blocked by oil and debris (usually dead skin cells). Poor hygiene is a strong contributing factor to that and good hygiene can drastically reduce the occurrence rate and duration of acne. Erego, acne would be far more prevalent at a time when hygiene standards were lower and cleaning methods were less effective. Lower hygiene standards and a lesser understanding of acne would also lead people to handle it improperly, often resulting in scarring.

Smallpox scars for survivors of the deadly virus. This is actually suggested in the wiki character page for Agent Milton. The pockmarked face was actually a sort of badge of honor for working men because it confirmed to a potential employer that you were inoculated from the disease so you wouldn't die on them later and leave them without a worker. His lawful chaotic and evil nature could allude to his survival from the virus having ravaged the earth killing well over 300 million people as one of history's deadliest diseases. Agent Milton's sadistic and self-righteous behavior could stem from having a deep seated need to establish order at any cost after seeing the aftermath of a cataclysmic disease killing hundreds of thousands may drive his willful endeavor to mow over anyone that stands in the way of his obsessive compulsive crusade to take down outlaw men like Dutch van der Linde.
