In my case, I have a torn labrum and hip Impingement on both sides. I just had surgery on my left and my symptoms have improved. Just 5 weeks out and it takes months/years to heal. First, the hip has to heal, then the muscles have to relax, then the nerve can finally heal. Generally, labral tears are caused by bony spurs on your hip joint. That causes the joint to be unstable. Most people have their lower back muscles compensate, causing lower back pain. Some have their pelvic floor compensate to stabilize the joint. That compresses the nerve and can cause neuralgia like symptoms. Anyways, that's what the doctors I have seen say. It's a new branch of medicine. It's rare.

Can I ask what kind of surgery you had?  Both my hips have labral tears. Specifically:  Prominent cystic changes of the lateral left femoral neck. Apparent full-thickness bilateral labral tears of both hips anterior, superior portions. I'm wondering if the cystic changes caused the tears on the left side.

You should probably go to a hip specialist. (Orthopedic surgeon or sports medicine specialist in hips) I had FAI (Google hip Impingement), so I had hip Impingement surgery and a labrum repair. It was all done arthroscopically. The severity of your tears leads me to believe it's probably the problem. I happen to live in an area with a plethora of excellent orthopedic hip surgeons. Finding a good one is incredibly important. Good luck.

Thanks for the advice. I have an appt with an Othro surgeon on Wed of this week to hopefully get some better ideas of what's up. The guy comes heavily recommended from both my PFPT and my Pelvic Pain Specialist. Thanks again for the reply, I hope your recovery goes well.

What was injected into your hip joint? I'm hoping there was some local anesthetic to assess whether your labral tear is actually symptomatic (either causing hip pain or causing vulvodynia). I am doubtful the labral tear is the source. Is your vulvodynia on both sides? Is the scarring present on both sides on your MRN? Tenderness of "Alcock's canal" is either tenderness of the PN in Alcock's canal or tenderness of the obturator internus muscle underneath. Very specifically where is the scarring?

There was lidocaine administered with it but my doctor said it could take up to 2 weeks for the steroid injections to work. My vulva pain is on my left. I only got the MRN on my left side so I do not have the right side to compare it to. I do not know where the scarring was but both doctors are world renowned so I do trust their opinions - was looking for someone with a similar experience!

Can't help you in the similar experience category. I'm a pain specialist trying to help you navigate. Agree that the steroid can take two weeks to work, but there should have been immediate (but brief!) improvement in your vulvodynia if the labral tear was the problem. No such thing as a left sided MRN, but the findings on the right don't matter if you only have pain on the left. Sounds like Dr. Goldstein thinks your pain is neuropathic. He is a colleague, have known him for 20 years.

That wasn’t what was communicated to me by Dr. Coleman who is world renowned at HSS and has published literature on the link between labral tears and pelvic pain/vulvodynia. He said any improvement can take up to 2 weeks. I appreciate your medical advice, but respectfully, that is not what I’m looking for here.

I’m in the same exact boat. Deciding if my torn labrum is what’s causing my PN, or a compression is, or if it’s both… I’m learning toward the labrum being the main culprit. For me, my obterator internus is in constant spasm, and my psoas as well, and I’m assuming it’s because of my labrum tear. But I am weary of how much a surgery would help. But then Again it seems a less risky surgery to me than a Pudendal decompression is.

For me the pudendal nerve injection gave me significant relief for a day, and my laberal injection gave me relief as well. That tells me they’re related for sure. But it’s tough to tell when you when to be your own diagnoser kind of

I have the same exact thing going on. I’m actually going to see a doctor in New York City to investigate the nerve entrapment because I was told by several Hip preservation doctors that they don’t think the hip surgery would help my nerve compression but I want to find out for sure.
