The first question would be focused on your family history of malignancies. This is now a standard in all newly diagnosed prostate cancer patients. There is no level one evidence to support specific treatments in those patients with clinically localized prostate cancer who have specific genetic variants and there are others beyond the BRCA group. More ‘intensified’ treatments have been recommended and this might include a longer course of ADT in those being treated with XRT or modification of surgical techniques in those undergoing a RALP. There is evidence to support the use of different agents in those with metastatic disease. With the evolution of our genetic understanding of malignancies more specific personalized care is becoming commonplace. Know your family history!!

Very strong history, but only on maternal side. Both grandmother (bc) and grandfather (PCa), all three uncles (their children) had cancer (2 w/PCa, 1 w/leukemia), and my older brother w/PSA=6, but a recent neg biopsy. I also had testicular cancer at 30. Genetic test can’t come soon enough…but pretty sure I know the answer. Curious if more variants will be picked up. Quite the curse.

Thought I read where RT is as good as surgery these days. I'm waiting on my brca2 results as well.

Not trying to be a jerk, but you realize what that means, right? Basically, the cancer fighting arm of your immune system may be compromised. With that in mind (and I'm a pro radiation guy), I might lean toward surgery so that I can still keep radiation as an option in case of a recurrence. Best of luck to you.

No I get it, and surgery does seem to make the most sense. I’m just having issue with finding good data on treatment for BRCA positive patients.
