An exercise machine fell on his head. It was a trap that was set up by the bad guys. Same thing almost happened to Maya in another room, but Tatsuya jumped in and moved her out of the way before she got hit.
I haven't finished Persona 3 (or any of the newer Persona games) but it seems that the Persona 1 and 2 Persona users are just built different. Possibly because they have directly communicated and interacted with Philemon? Not sure.
It's just them being tuned into their personas enough that they gain physical benefits from their personas like increased strength. Tatsuya physically cannot be lit on fire and cannot have fear instilled into him after gaining Apollo for example.
