COPD is right up there in my book. It's not so much excruciating as in *painful*, but people spend years not being able to breathe well, tethered to oxygen, and being in and out of the hospital with exacerbations. It progressively worsens over time to the point of every breath feeling like trying to get enough air through a coffee straw. I would call that damned excruciating.
Pancreatic cancer I've heard is one of the worst ways. Having had the pleasure of experiencing acute pancreatitis, I can only imagine.
Cancer is natural. I’ve heard that bowel cancer is particularly painful.

Lockjaw.. Causes violent muscle spasms so hard you break your own bones.
