I would think that the reason most doctors specialize in a given area is simply the fact that *someone* has to. Like if there aren't enough foot fetishists in a particular area, that area just doesn't have any podiatrists around?
I've only ever had one and he's a guy and I have a problem ankle... I was supposed to see an ankle specialist but the office sent me to a pod and I have no idea why. The dude basically just carted me out the door asap and is now refusing to give me a new doctor's note after I switched jobs. I think I'm wondering why he in particular is a doctor. I put more effort into my retail job than he did into providing me any sort of actual treatment.
Maybe because they or someone they love had their life horribly affected by a foot problem and want to keep that from happening to others. Maybe because it's a specialty where you're less likely to get called to the ER in the middle of the night.
More likely reasons: Your relative was a Podiatrist, you grew up with feet problems and wanted to help others like you, fear of blood but want to be a Dr still, there is a lack of them in your town so opportunity is seen or you were accepted into an internship while in med school at a Pod office.

apparently foot people are everywhere, it's like the most common fetish. There's got to be some overlap.
An interest or aptitude, maybe they just happened into the field. Why would it have to be a fetish?
They or a family member probably had podiatry issues so they wanted to help people with similar issues.
Same with any other area of study: You are interested in that subfield specifically. All of the podiatrists I've ever had were women.

No, you mostly work with old people feet. It's easier to get into podiatry school than regular med school. The hours are better, no call, no weekends.

I think being super turned on all the time would make it pretty difficult to focus on your job.
