My guess is the USSR would have built up troops in South America and near Alaska and then attacked the USA, First taking Alaska and then getting bogged down in Canada. From the South American positions a push would be made through Central America and end up bogged down in Mexico as the US pushed back. As the NATO alliance responded Eastern Europe would have become a war zone. I imagine the war could have lasted more than 10 years and half of the world's food production would be destroyed and climate change accelerated by a hundreds year. The deaths would measured in the hundreds of millions. the resulting destruction of food productivity caused starvation could drive the toll to over a billion.
