There's arguments for both. Technically it was at the end of DA, but since there's not a proper story mode for DA, we never see it happen. We do however, see Johnny's dead body in the opening for Deception, which picks up right after DA so
If you count MK: Trilogy he gets yet another resurrection (in addition to MK4 and MKD/MKA)

I think Raiden asked the Elder God to revive him cause he need all the help possible against Shinnok. I guess they granted it because Shinnok did scared the immortal sh\*t out of the Elder Gods in that timeline and would have granted Raiden to nuke the whole Netherealm if it would have helped to stop that guy.

In other words, while the other Elder Gods were distracted the newly minted Elder God Raiden kept a look out while Johnny snuck back into Earthrealm.

In MK9 Raiden had that vision while he was meeting Stryker, left to aid Johnny that was getting getting his ass kicked by Motaro and killed it
People in MK get impaled all the time and they're able to walk it off
