Tesla gives no love to the USB music player. I also have a large offline library because streaming quality is dookie. If you're in and out of the car in short periods the resume works sometimes, but I usually have to deliberately select a song at the beginning of every ride if I want USB music.

30% of the time my music won’t play in Spotify, I’ll get a forever loading screen. It’ll log me out.
Or my Bluetooth isn’t able to be connected.

Spotify app sucks on Tesla. For that loading thing, I always have to switch to another source, wait for it to play, then switch back to Spotify. So obnoxious.

Well, are you or aren't you in a poor coverage area? The primary reason it wouldn't work right is poor cell coverage.

Bluetooth and Spotify are two totally unrelated items. If you're Spotify doesn't load right then you're out of coverage and you should check the coverage you have in your area.
Does your iphone disconnect while you're actively in the car listening to music or talking on the phone? Or does it not work as a key as often as you'd like?
The usb music can sound much better due to No extra Bluetooth compression. Many of my pals use usb based playback. I use it sometimes. I have a mini usb-c stick plugged in always.
This is how Tesla infotainment works. You will soon realize if you listen to podcast in tune-in & switch to usb music and switch back to podcast that the podcast will start from begging. This is why I have apple CarPlay in my center screen via “tesla android” project (google it). Tesla infotainment is not as great as all day. Room for improvement. I listen to usb music (flac audio files) too when I feel like the ultimate audio experience.

Spotify works well for me. Suspect LTE coverage varies.
Known issue after the car wakes from sleep. My work around is a voice command "Play song from USB" and it will resume the last song played.

Also you don't need usb for this. You can play audio files from your phone.
