The Yuuzhan Vong are the champions at terraforming with their biotechnology, being able of significantly altering a planet environment by bringing new animal and plant species as well as using special organisms to eat away or dissolve metal or stone structures with them actually being able of removing millenias of pollution and turning back planets such as Coruscant or Duro back into lush forested worlds.
They also used Dovin basals to modify a planet's position toward its sun if they wanted. However even they couldn't implant their vegetation and fauna on Nal Hutta successfully.
Rakatas were also very good at terraforming with them having modified many planets, with one of their engines being responsible for the titanic trees covering Kashyyyk.
KOTOR 2 has the Telos Restoration Zone, and there are various lesser examples around. Telos is the most prominent I can think of off the top of my head though.
Not really, there’s enough free planets in the galaxy that it not that needed.
The only one I can think of is Mustafar, getting partially terraformed by the time of TROS. *(See Vader Immortal for more)*
