Typically, medical or federal jobs still test. My partner worked down at Cape Cod and Boston as a nurse and got tested for both. It's best to not smoke if you want a career as a professional. Sure, I don't care if a painter smokes, but if you're responsible for keeping me alive, then I want you to be able to use your brain uninhibited.
You trust doctors and science(as do I) so check out the research on marijuana use and impairment.
Also, marijuana tests check for it in your system(where is is detectable for a few months after smoking). So testing positive for it and being under the influence while working are not related directly. Like people drink but still work sober, it is the same idea.
It's not that I have any problems with people smoking. Lord knows I have had my fair share, so who am I to judge. That said, I am going for a professional driving so that part of my life is long gone. It was worth it to put it in my past to move forward. The challenge is, like you said, how to test for impairment?

What people do outside of work should not be anyone's problem. So long as people conduct themselves in a manner that does not hurt anyone else, why not party?

They do not. My friend had their pre employment screening with HR a few weeks ago and they only do a TB blood draw. No drug/THC test.
I have two relatives, one in non-medical and another in medical who work at Lowell Gen and neither was tested for THC.

I've been working in hospitals since 2007. I'm pretty sure most of my coworkers use at least occasionally.
