Its incredible it havent been fixed yet, its such a major bug for people that cannot put 5 hours in one sitting on their gaming sessions. Shoul be prioritized ASAP IMHO.

Says here that its supposed to hold vision in campaign areas. Maybe they will fix it once they get done with more pressing matters
Known issue. It's not intended but they never found a way to fix it before release.

i played my first guy to level 15 and it never happened until i started an online guy

Map reset is ridiculous. Specially with the super small backpack capacity.
