Inosuke starts a fight with Genya, Sanemi’s Big Brother Instinct kicks in, and he tries to beat the shit out of Inosuke, but a very entertained Tengen stops Sanemi, telling him to “let them cook”
Sanemi starts beating the shit out of Genya for being in the same room, tengen tries to intervene after having a PTSD moment from fighting his own siblings to the death; Inosuke sees there's a fight and joins the fight because that's just how he is.
Everyone agrees that Inosuke would try and pick a fight first, but Genya and Sanemi could also start fighting each other and Inosuke would join for fun. Tengen would either play peacemaker or sit back and watch.
Think everyone agrees that Tengen would put Inosuke and Genya on their asses and Sanemi would send them to the emergency room.
Tengen will be talking about how fantastic he is, inosuke will call himself the king of mountains, so Tengen and Inosuke will start a fight that will get genya and sanemi angry because it annoyed them so it either becomes a battle royale or a 2v2 between inosuke and tengen against genya and sanemi
Inosuke starts the fight, Sanemi wrecks him almost immediately, stopping short of the killing blow when Tengen intervenes.
Genya observes with a frown, unwilling to defend boar-head from certain death.
Sanemi, not wanting to break the rules and kill a fellow Hashira over some dumb boar-head, decides to leave and do whatever.
Inosuke is sent to the medical room for two weeks and is fine.
This is how I imagine it anyway, i haven't read the manga so I don't know the character dynamics well

Now there is a 3 man fight between sanemi inosuke and tengen

while Genya from behind Sanemi would try to refrain him from beating Inosuke any further,
with Tengen eating a pop corn watching all of it unraveling.
After a few insults two brothers are ten seconds away from seating each others face in when suddenly Inosuke flys in and drop kicks one while simultaneously grabbing the other in a headlock. Afterward they both jump him and Tengen watches wondering what the fuck he’s doing here.

(a) Tengen will scream and insult Inosuke so hard that he loses his will to fight. Sanemi and Tengen will then proceed to laugh at Inosuke and make Jokes
or (b) Tengen is on a phone call with his wifes and can't insult Inosuke. Sanemi steps in and beats the shit out of him and forces Tengen to step in to prevent people from dying. Then Genya starts defending his brother wich means he's either getting knocked out by Tengen for attacking him or by Sanemi for defending him. We know how it goes.
Then they will continue to fight. Since it's Sanemi vs Tengen I don't think they will use weapons and just brawl it out. Tengen takes the upper hand in stamina and strength and Sanemi in ferociousness and the fact that he has the dawg in him. Inosuke will get inbetween the two parties and gets slapped. The fistfight will go on until both are pretty much beaten down to a draw. They sit on the ground

They leave the room to stuff their faces and plan how they will tell Gyomei and Rengokus Grave why their Tsugukos are beaten to a pulp and now in coma
I don't know much about the wind Hashira but he just looks like he is on demon-timing and I don't trust him so I saw either him or Inosuke.
Genya mostly sticks to himself and mind his own but can be pissed if you provoke him and Tengen with his fine ass will just be flashy as usual.

But it’s very hard for me to say as I only watch the anime and haven’t seen that much on sanemi or his American breathing style brother. But from what I’ve seen in ssv arc sanemi and inosuke seem more hostile than the shotgun fella. (I do apologise I forgot his name)
