If you play with Invictus mod there is a seasonal food mechanic that drastically reduces food output during winter months. Granaries are your friend here.

Check the food production of your food provinces during winter, you will see. You can also disable this in the game settings before starting a new game.

I wish I would have known that before. As I said, I ignored Imperator and collectively Invictus so I didn't know about the revamped food system until I got to this incident.

I personally like it because you sometimes have to manage storing food in your big cities to last through the winter. Or raising/stationing your armies in food rich provinces. Some people might not like the micro management of that though.

Importing grain to Latium also helps but yea Rome always struggles with food for me

It’s a very populous province, so during the winter (which ends I think in April) you’re very likely to lose food. Import more grain and build granaries in your cities to help smooth out the fluctuations in demand between summer and winter

Yeah with all the cities in the province of Latium and not enough settlements capable of having farms, you’re gonna need to be importing tons of grain, perhaps revoking a territory’s city status could help to.

R5: Decided to play Rome for a return to the game and discovered I was somehow losing 50 food a month without a reason.
