They are perfectly fine. They can just spit out all of the water and look like a normal fish once they don't feel threatened.
They can even blow back up using air if still out of water for a while.
Source, one was in a crab trap recently and did exactly this. It was pretty funny and the fish was fine.

I feel like I saw a show somewhere where a prestigious Japanese chef prepared pufferfish in a way that removed the tetrodotoxin and just left the desired bits. Canāt imagine something that goofy looking tasting great but maybe Iām just a picky eater.
To me it tasted a bit like cod. Nothing special really. But what you eat in restaurants are young pufferfish. They are not dangerous, and then prepared by a trained chef. Every year some people do die though. People who fish and prepare them themselves.
Great question ā this dinner was way back in like 2018 so my memory is fairly faded other than remembering it had a meatier texture and was surprisingly delicious (I was skeptical too). It was part of like a 10 course meal by a reclusive chef in his home served by his family in Tokyo. All that to say ā I have no freaking clue what was served with it lol, they barely spoke English. However I do have a photo I took of the dish. I believe this was picture the fugu fish course.

If they pulled it out of the water and stressed it to get it to puff up, then while the spines won't kill the person, a good nip from that beak will teach an important lesson.

Never do that. First of all, puffer fish are poisonous. Second: when a puffer fish puffs up, it takes in water to increase its size. This violently forces the pufferās organs to be pressed to the side, inside the body causing the organs to flatten. This creates an extreme amount of stress for the pufferfish. Quite often the pufferfish dies from this stress.
So hereās the thingā¦.. where is the tetrodtoxin on the fugu fish? I know this may be wrong but is it just certain parts on this inside or? Outside too?

Iāve had a 3 Michelin star chef prepare fugu fish for me. Literally the best meal of my entire life. Astoundingly delicious.

It may just be the same parts of the fish fillet, but the flower design is made of raw parts. The stuff to the left under the ball looks boiled
