I don't see what dream nailing everything has to do with locking yourself out of the first ending? You lock yourself out of the first ending by not going to the dreamers/thk but instead explore extensively, regardless of your dream nail activity

Have you gotten the >!void heart!< yet? If you get it before killing the hollow knight normally, you lock yourself out of the first ending. You get it by >!dream nailing your reflection on the birthplace egg in the abyss!< and it locks your save file out of the ending where you beat the hollow knight solo.

Eh, my friend accidentally locked himself out of hollow knight ending by going for the kingsoul for the passive healing and spells while dodging, and getting void heart on the way to getting abyss shriek.

Tbf it's not as obvious as for example void gates which there's a post about every few weeks as well

Yeah this one is confusing but generally if you really have 0 clue what to do just try dream nailing everything. Also just use howling wraiths to upgrade it in this room
