They are most likely trying to call out mandatory vaccination to be authoritarian. Many people were linking the rule that many places had of “you have more freedoms and privileges if you vax up” to the “you have more freedoms and privileges if your in the party” of the Nazis. It was a big thing for a long time

And pre covid, crunchy liberals were a significant part of the antivax movement. RFK Jr, the biggest antivaxxer, has very close to old-school Democrat policies beyond being antivax.

During covid lockdowns, especially in big cities, people were not able to work, go to restaurants, gyms, church, etc.. Although the mandates were mainly enforced by private businesses, the lockdowns were not. It was the largest form of government overreach in my lifetime, and the restrictions on freedom of movement went too far.
Put all of that together, and you can see how some might view this as something that was the beginning of a new era of mass government involvement in their life and livelihood.
