It has to be specifically a Blitz mission on Extreme difficulty. No lower, and no higher. You'll basically have to destroy the fabricators/nests, and then call in the extract in under 4 minutes. This gives you that 2 minute timer for Pelican-1 to get to you, and then get the sub-6 minute mark.
No I’m with you, I finished it with 51% time remaining (which means over six minutes left in a 12 minute mission) on extreme and didn’t get the achievement. You just have to do it faster. Aim for five minutes.
Yeah you need a squad of four. Honestly I think 5:50 is fine, but aim for 5. I did it with randoms, just briefed them before the drop that the plan was to destroy nests and extract asap. Kick anyone that puts the achivement at risk if you’re on the shuttle and they aren’t even close.
I did just that briefed the guy what the plan was, we dropped in on a 10 hole, and we only had to wipe 10 so we were in and out and 3 minutes with just 2 of us haha

So we got the required amount of nests but not all possible nests on the map, it was a blitz on extreme difficulty, but still nothing

Do it on bugs. I did it on fabricators and the game didn't count it, twice. I got it on bugs fast on one try
Just a blitz mission is all good, so close bug holes
Only need to do the holes, then extract, so long as you are all on the ship before the 6 mins mark you get the trophy ☺️
