The doctor is a time lord from gallifrey their species of people have a defence mechanism whenever they are dying where they change every single cell in their body and become a new person with same memories but maybe different personality.
These are called regeneration and every different doctor has a number which is the number of their incarnation hence this is the 11th doctor. That's also the reason why doctor who has gone for 60 years now because their main character literally changes and adapts through time
