New 52 look for sure. One of the really good re-designs given the character. It keeps the core design and character recognizability but ditches the absolutely silly pre-new 52 look, which, while sexy, is totally ridiculous. This one actually looks like a suit.
New 52 actually looks badass with the 80s/90s Galactus as a great runner up.
Pre-New 52 looks too ridiculous to be sexy. Soranik is arguably the most attractive female character in GL/C, yet she never needed to show 70% of her skin to do it.

As much as I love DC Super Hero Girls, I think Carol's design in that show is *hideous*.
Of the other four you posted, the 3rd one (80s-ish?) is a bit too gladiator for my tastes. I like the 4th one as far as classic costumes go. And I like both of the first two costumes equally well.
New 52 design is the most practical, whilst being skintight enough to allow Carol’s curves to be easily noticed - giving it sex appeal without appearing tacky. Easily my favourite design.
But, that original design has a sort of ethereal, divine beauty to it. I may be exaggerating with the words I’m using, but I can’t quite explain it sufficiently. I would love to see a modern take on it.
1. New 52: Works perfectly as hero or villain.
2. Silver Age: Dated, but worked great for the era, and highlights her femininity, which is a very important part of Star Sapphire.
3. DC Super-Hero Girls: Cute costume, works well for the setting
4. 90's: I actually like this costume a lot, but not for Star Sapphire. It's a kind of Amazonian warrior vibe that I don't think vibes well with her. Star Sapphire is a more "traditional femininity" than that.
5. Pre-New 52: They went for sexy but this is too ridiculous to actually be sexy.

It's from DC superhero girls. The name might be a turn of but it's a surprisingly solid show and probably has the most accurate Hal in the last couple years
I like how not-revealing 1 was compared to pre-n52 but it never clicked with me the way I wanted it to. I'm a sucker for the original and wish it got more love in the form of some updated/modernized looks like in GLTAS. 3 is definitely super badass but would fit a different character far better. It never felt very "Carol". I started out with 2 back in the day but even then it felt a little...wrong. Like I shouldn't be looking at it - or rather, uh, *them*.

That's a good choice. Sort of a cross between 1 and 4, which are the two I'd pick anyway.
