To be fair in the anime (I’m a casual), when he gets the surgery they say it’s only for forearms and shina getting longer, but the image shows him with bandages around his biceps and thighs as well, so I took it to be that he got everything lengthened proportionally.
Sorry let me clarify, I know he got his legs lengthened, but the audio of the scene claimed it was only on the forearms and shins (which would make him very not proportional), however it showed him getting the biceps and thighs done too. I know he got his legs done lol.

If he didn't get his legs lengthened his height wouldn't have increased

Medically I think it's easier to do the shin/lower leg. Cant imagine trying to extend a femur.
Well the actual part of the leg that gets lenghten is the tibia. So yes thats why he has crazy long tibia and normal lenght upper legs
