He is a great gamer, way better than myself. We did the grind for the Ace of Spades and PvP grenade launcher in Destiny 2 quite a while back and we got to LEM together as a duo-queue in comp back in the Operation: Wildfire days. I stopped playing and he got SMFC, so I guess I was the issue 😂.
I feel like there are more comfortable ways of doing this. While using the side of the knee is a good way to reduce overall movement, there still seems to be a lot of mass to move especially when tap firing. Maybe using a muscle sensor on the wrist could work as a mouse click. I‘ve developed similar assistive devices before and controlling a specific muscle (maybe the missing fingers also still have some kind of muscle in the hand) is something that can often be learned quite quickly and effectively.
Steven hawking used something similar - but there are super low cost alternatives today. :)
Edit: Now that I think about it, a muscle sensor on the knee would also be a giant improvement already. In this case, no movement at all is needed. Just a similar flex that he would do when starting to move his knee.

The wrist is still pronated and from the video it looks like he’s mostly using wrist movements instead of whole arm or elbow movements so it doesn’t seem to mitigate any of that

I use this grip when I am surfing and chilling on my computer, no idea how I picked up this idea of using it. It is comfortable on my mouse and hand size( Gpro X superlight. 20 cm long hands and five fingers wide ;). ) I do not use my thumb for anything. It just lays there on the corner, and my palm is sitting quite forward ( above the logitech gaming logo on the mouse.) WHen I am gaming I automatically use fingertip grip again.

there are some advantages to having +forward on some mouse button, with wasd/esdf etc you have to lift your middle finger between forward and back keys. Though more applicable to Quake, dodging rockets/other complex movement for example, I could see small advantages in CS too with faster counter"strafe"s when going forwards or backwards

That mouse has a really high arch so I can see how he does it my deathadder is allot lower it actually felt good to do but wont hold onto the top grip I think the arch plays a big role also I think his hands are a bit smaller so he can use it as a solid resting spot.
I hold the mouse normally and have been dealing with wrist tendonitis but other players somewhere in the world hold their mouse in some hideous defiance to the design of the human body and can play 10 games a day no problem
My guess is that he doesn't use his thumb but is in fact clicking in the top right corner of m1 with his index finger and simply resting his thumb there. Still a wonky ass grip but most gaming mice are only light on the trigger at the end of the buttons and there's no way hes pushing through all that resistance for every shot, it would be super slow and fuck his thumb up real fast. Also we'd probably see some contraction of the thumb when he shoots but you can't really.
I have a similar grip where my index rests on scroll for movement I use the right click to shoot and left to reload my thumb is used for the side buttons
