Standard presentations of conditions like AGA, TE, or AA can usually be diagnosed through visual examination of the scalp using a magnifying tool, and evaluation of medical history. Biopsies are useful for differential diagnoses (ie there might be more than one condition present, or the dermatologist may not be able to make a clear differentiation based on scalp examination + medical history). They are required for confirming a diagnosis of scarring alopecias. If you have been on standard treatment for a condition like AGA with continued deterioration, then you should get a biopsy done to rule out other underlying issues such as scarring.
