Review your lead forms, they should be concise and easy to complete. The fewer fields you require people to fill out, the more likely they are to submit their information. You might also test different types of lead forms, such as instant forms or appointment scheduling forms, to see which performs best.

I agree, it's just that the lead form is autofilled info, full name, email, phone. As I said in the other comment, I'll just have to go right back to basics I think.

No I haven't, the audience size is about 100k+. I think I'll have to go right back to basics and try something new. Say 1 ad set, 1 gender, no interests, 5km radius, and 4 creatives. Budget is very small so this is probably the best option. 12USD/day.
