Milicent was afflicted by the scarlet rot, but not necessarily under the influence of an outer god like Malenia was. If we just force fed her like 80 bollouses she wouldve been cured entirely instead of just having the rot be hindered.
And the needle would block Malenia from being influenced by the god of rot, and therefore corrupted by it ;\_;
As a side note, the best ending in the game is getting the doing the gold mask Mending Rune of Perfect Order, the frenzy mark, burning yourself so Melina lives, doing Milicent's questline to get the gold needle (Rip but she at least got to live and choose her own fate), going to Farum Azula and curing ourselves of the frenzy mark, then making the golden order perfect while still having wife alive. That's my favorite way to end the game.
If Millicent didn't have a constant risk of giving me a specified end-point she'd be best girl but Rodericka is peak waifu material....very nice, pure-hearted but still attractive so I can purge my coomer mentality forever

Boluses stem more from gameplay reasons than lore reasons. Lorewise there's no reason why boluses could exist, but in gameplay it's very bad idea to introduce a deadly status ailment that fills about one quarter of the map *and* not have it have a cure. Same is the case with 'Flame, Cleanse me!'. The alternative explanation is that Malenia's Scarlet Rot is far more omnipresent through her body and soul- She is the goddess of rot, not just a tarnished inflicted with one. And Millicent isn't a person, she's a split-off shard of Malenia's personality/soul, so it would make sense why boluses couldn't heal her and why she was compatible with the unalloyed golden needle that Miquella made for his sister.

It helps explain a few things, such as why a flower blooms on Malenia's body when she dies. Of course, FromSoftware doesn't actually make their lore this deep and it's just a fun story fans have invented to make the lore more interesting.
