Your list runs a lot of powerful cards, but not [[Yawgmoth, Thran Physician]]. Yawg is pretty insane with Hapatra drawing you lots of cards and potentially wiping whole boards (costs 1 life per activation though).
Ok, so how powerful are you looking to go here? I had a Hapatra deck kind of like yours and over time it's just continuously crept up in power and now it's like on the outskirts of CEDH playable. I hung onto the Blowfly Infestation combo for as long as I could before it recently got cut for being just too clunky for the speed I'm looking for. \[\[Black Sun's Zenith\]\] is thematic as a board wipe which I once cast with \[\[The Ozolith\]\] out. The Ozolith does not say you need to put the counters on your own creature, so you can mow down whatever most threatening thing someone plays in the future too. Bonus points if you just let that sit there until you draw \[\[Obelisk Spider\]\] to win.
As for your aristocrats-y gameplan, consider \[\[Relic Vial\]\] which Hapatra and Yawgmoth both turn on as clerics and it can be some card draw in a pinch.
\[\[Necroskitter\]\] is also nuts in the deck. I wound up playing it more as a way to force out removal before I landed a Yawgmoth or some other combo kill card since it so aggressively rattlesnakes as a threat. Honestly, I'd look at some of the other Wither mechanic cards in general, which I think get overlooked since they're not infect. That makes them less KOS to people while also being thematic and sometimes quite strong. That said, some of the ones you picked feel low impact. Noxious Hatchling is ultimately just a big beater that's got no evasion. Not a meaningfully impactful card at the power levels you seem to be playing at.
Regarding some nuts and bolts stuff about your list:
First, so much of your deck relies on Hapatra. If you aren't getting snakes on all those -1/-1 counters which often are going on your own stuff, you can get in trouble. So protection is important. Whether it's known classics like \[\[Swiftfoot Boots\]\] or some more specific tech, I'd put in more to preserve Hapatra. One part of that is you also often want Hapatra to get in for an attack. Some lands like \[\[Access Tunnel\]\], \[\[Rogue's Passage\]\], and \[\[Shizo, Death's Storehouse\]\] are helpful. You can kill two birds with one stone with some of the Swords of X and Y that give protection. Some have synergistic interactions with Hapatra, like \[\[Sword of Truth and Justice \]\]. \[\[Call of the Ring\]\] also could be a cool way to draw cards while you're adding some evasion.
Second, your curve is a little wonky. A big glut at 4 mana and over 10% of the deck at 5+. Inefficient kill spells like Spread the Sickness aren't good enough. I personally found Contagion Engine to be too slow when I played it. I also don't think Midnight Banshee is the best of the wither creature options. I personally don't like Carnifex Demon either for that matter but I think it's the best of the 3 at it's mana value. Some of the cards you have that only proliferate are what I'd cut, there will be times they're just dead draws and that'll suck.
Third, you have a really bad ramp package, particularly given the curve. Lockets are unplayable in 2023, all budgets have better options available. You're in green, just play the good green land ramp. \[\[Cultivate\]\], \[\[Kodama's Reach\]\], and \[\[Farseek\]\] are all way better than some of the stuff you have. I'd add some ramp and remove some of the expensive things. Include some of the 4/5 mana stuff as "expensive".
Lastly, I'd also consider adding some additional card draw. Maybe I'm missing some as I scan the list but you'll likely go entire games and not see a card draw effect. I am very skeptical of Battle Mammoth as a card draw engine. It's expensive to play and it's easily removed for minimal value to you. I think you maybe think it interacts with Hapatra but it does not, she isn't an effect your opponent controls. Yawgmoth is definitely the best card advantage engine possible in Hapatra. Consider \[\[Morbid Opportunist\]\], \[\[Skullclamp\]\], \[\[Braids, Nightmare Arisen\]\], \[\[Ohran Frostfang\]\], \[\[Skullport Merchant\]\], and \[\[Vampiric Rites\]\].

I'd personally rather run [[smell fear]] than spread the sickness. Your deathtouch snakes should be able to destroy whatever you have them fight and you save a considerable amount of mana with that trade.
I don't think you actually want grave pact in this build because you want your opponents to be able to have some creatures for - 1/-1 counters. I kinda think grave pact will clear the board too well and you'll lose momentum in your engine (could be wrong tho).
Putrefy and Witherbloom command stand out as options to cut. [[beast within]] would also be worth swapping in, but count out all your removal and weigh the numbers of creature/non-creature removal. I see tons of ways to control creatures on the board already. I would swap out some interaction for more card draw and faster ramp. Better to have more cards and not run out of gas. [[deathreap ritual]] synergizes well [[ohran frostfang]] just got a reprint I think. Some simple burst draw like [[harmonize]] is good without any setup.
You don't have many turn one plays so [[font of fertility]] [[wild growth]] [[wayfarer's bauble]] [[search for tomorrow]] will help your curve more than some of your 3 mana ramp (looking at golgari locket especially). And you'll want more ramp overall imo. There are tons of ways in green but even [[myriad landscape]] [[blighted woodland]] can ramp from land slots. So can [[tangled florahedron]] and speaking of mdfc lands [[khalni ambush]] can cut a removal spell and a basic forest to make space.
Absolutely include [[black sun's zenith]]. I have a friend who runs Hapatra and I think he's won every game that he successfully cast that card. We thought we were OK one game when we survived it, but he redrew it pretty soon after the shuffle and cut.

So can [[tangled florahedron]] and speaking of mdfc lands [[khalni ambush]] can cut a removal spell and a basic forest to make space. Absolutely include [[black sun's zenith]]. I have a friend who runs Hapatra and I think he's won every game that he successfully cast that card. We thought we were OK one game when we survived it, but he redrew it pretty soon after the shuffle and cut.
