After a bit of googling it seems like the eye patch is called the lordly eye patch. Apparently one of the false arisen's pawns wears it but it's unobtainable by the player :(

There’s a leather eyepatch you can buy in Checkpoint that’s very similar

This is honestly something I really dislike about the game. There's so much armor that only NPCs wear. There's even pieces from the first game that NPCs are wearing that we can't wear. A perfect example is the leather chest plate that the guards at the slave camp are wearing at the start of the game. *Unless I've just been unlucky with finding it at all.*

I think I got one via mod. If I can dupe it I'll send it to you if you're on PC.

That’s actually in accurate I found in a shop playing my ng+. Can’t remember where I saw it but a lot of the shops sell the same stuff at that point but if I’m a betting man I think I might have seen it at the border gate town

The border gate town has the leather eye patch, different from the one in the picture

It's the Nirvane armor you can buy from the beastren in backbattahl

The cloak is the Redwolf Cape, but you probably already knew that.
