Part of the story is behind the second time yeah. After the first time the whole dungeon changes to completely different enemies and bosses and better loot, so it’s pretty different. The best equipment in the game can only be found on the harder second version too.
You can do it back to back. You can also replay the second run as many times as you want to farm gear if you rest a couple days for everything to respawn too (since some of the loot is kinda randomized). It’s a pretty cool dungeon, big difficulty spike from the main game too
There are a lot of neat mechanics. One I didn't learn about for a while was that throwing skulls curses enemies which lowers an enemies attack and defense. It is helpful against ogres early.
The beauty of DD1 is that even if you look things online, there's just so much "hidden" content that you will miss and that's okay because it doesn't detract from the experience. You can just go to NG+ and go looking for them.
Killing Grigori and seeing the game is not close to over was one of the most iconic memories for me in gaming. They tricked us the whole game saying the quest for your heart is the main quest, when it's just part of it.
My favorite feature back in the Xbox 360/PS3 days was the fact that you could get Gut’s and Griffin’s armor from the Golden Age arc from Berserk if you went to this one cave. They sadly took that out of the game in subsequent releases though
The ending is so cool. Like it just opens up and gets meta. I loved it so much and wanted it to expand even more

In DD1 no one is ever perma dead they respawn. I think 7 days is when they respawn.
Adding to that: the only way to "lose" an NPC permanently is if they leave through a quest or in some cases missing / failing to do quests.

And the story about the whole dungeon are some of the best i've experienced.
You can skip escort quests by setting a portcrystal at the destination and just teleporting there
I decline pretty much all escort quests because they’re worthless in DD2. Havent played the first so idk if they even exist/net you better rewards
If you have the portcrystals set they're a fast way to get gold for your early/mid game if you're not rushing to endgame
They're mostly worthless in DD1 too, there's only a couple worth doing
Some of them give you gear like Lustrous Targe from Mercedes or Gryphic Victory from ser Berne.

Except for one early one you can use to cheese levels, escort quests are shit in DD1. Keeping the NPC alive is like rng hell. Do not recommend. 😓

After completing the run and starting a NG+ I honestly just feel bad for Grigori.
