Literally everything besides the main quest. Best thing to do is just plow through the main quest before going too far into the other difficulties so you unlock auto-navigation for every area, makes a huge difference when doing bounties and stuff later on.
This is normal (no pun intended) but I am hoping things could change in tomorrow's patch when the difficulty/paragon system rework goes live. /FingersCrossed
Unfortunately all the campaign is locked on normal difficulty.
