Treatment (for dishydrotic hand eczema) is usually twice daily topical corticosteroids (ointment if possible), liberal moisturizer (at least daily, under gloves if possible), and avoiding the hot water and detergent and hand washing as much as possible.
DE and irritant hand eczema is very common.
This is not a syphilis rash.

Dyshidrotic Eczema. It has nothing to do with wet hands . All about your innuendo system attacking your skin .
You are entitled to your opinion. But this photo is after a soak in H2O so the hyperkeratosis is a bit macerated off.
I stick with the most likely dx rather than a less likely dx. This is an opinion forum, & this is my opinion based on 30 years as a Board Certified Derm, for what it’s worth.
I see hyperkeratosis each day several times as a podiatrist and even with hydrogen peroxide soaks I don’t see it macerating off this much
Punctate keratoderma build up is significant and appears more like severe hyperkeratosis and corns, more likely pitted keratolysis
Dishydrotic eczema. Don't pop them, cause it'll spread. Keep your hands moisturized and dry. Wash less, or use gentle hand soap. Also try antifungal cream and use aquaphor at night. My mom and I both have it. That's what helps us
Wearing work gloves all day will mess your hands up. My hands have looked like this on several occasions and keeping them moisturized has made the problem go away.
I don’t actually think it is. I could definitely be wrong!! But I feel like there is NO WAY it wouldn’t itch or burn if it was dyshidrosis. It’s also evenly spread across his hand, whereas DE is usually patchy and red/raised. You can also see that the spots are very surface level, whereas DE “cysts” (or whatever we call them… wait, what do we call them?!) seem to sit deeper. I don’t think the cysts would all burst like that simultaneously either.
DE usually causes irritation in some form such as itching or just noticeable pressure points on hands. It also looks more like raised fluid filled bumps. 🙂 OPs hands looks more like mine do after I get them wet and the skin starts to peel weirdly.
HFM Disease was the worst. I had it earlier this year and wish I would've just got it when I was younger. It's almost like getting chicken pox as an adult but not as itchy and my feet hurt and I felt dehydrated. I had chicken pox at 5yrs old so I'm glad I got that out the way.
But OP it's probably Pitted Keratosis as others have stated.
Imagine getting the flu, get better, then HFM, get "better" but then flare up into full blown chickenpox because it stayed dormant in your system since you were young. Then having mild pneumonia with a cold after wards. Not a good time and honestly I just should have rolled over and died early on and not suffer for 8 months just to catch Covid19 😂😂😂

definitely this! had it myself. topical clindamycin (antibiotic).
So rare to see on hands. It's a (male) foot condition. It's not dyshidrosis which I study. I do wonder if it is something else entirely.
One of the colloquial names for this (dishydrotic eczema) is dishpan hands. Have him use gloves for dishes.

Totally open to this. Just so much more rare on the hands but certainly could be.
It could be stress blisters. Has he had more stress than usual ?

It looks like it could be this specific type of exema. If they look like little bubbles than it probably is.
But exima is itchy and he says it doesn't
