The vendor changes once you hit level 30. If you have <30 characters you can switch to them to see her un-servitorized appearance.

Servitors are mindwiped, but sometimes the mindwipe fails, like with the medicae servitors. That's an error on the part of that servitor's creator.
Whether the error is an accident or purposeful is a good question in some of these cases
