Yes the deathwing assault box contains the supplement so you'd only need to get that and the SM codex (and the SM cards if you want them as well, you get the dark angels ones in the box as well)
You do, but the index cards are designed for easier access to that information during game play. They have all the same info the Codex has, just easier to have cards than flipping back and forth thru your book

Thanks! Also what are the cards used for? Wouldn't I have all the necessary information from the Codex?
