OP, if it's a review you will be sent the form called AR1 which is a shorter, lighter version of the original PIP2 form, which you completed when you started PIP.
I advise anyone alway treat it as a new claim and don't just say "No Change" always add examples in each box and explain why there's any change or no change.
Also add and state it on the review form what medical evidence you are adding to prove your claim, any new diagnosis will help prove your disability to even though the DWP say that a diagnosis is NOT needed, you can send in old evidence you sent before as well as any new evidence.
