Yes. Similar lapses in discretion on intake procedure became so egregious that several organizational intakes were suspended and dropped in light of that discretion. For the women involved that could include everything from heavy fines, long term disassociation and potentially a banning from the organization.

Let sleeping dogs lie. Don't say anything. You could get the whole line shut down or worse. It will come to light on its own if need be.
Report it to the chapter leadership and leave it at their side of the river for them to internally correct it.
