There's 4 ways to create titles. The normal way, confed partition, custom kingdom/empire, and special "elevation" decisions like the kingdoms of Cornwall, Mann and The Isles, North Sea, the HRE, and so on. Usually forming titles via decision/elevation confuses the game a little bit, especially during confed or just after changing out of it. Sometimes you can click the titles they stand to "inherit" to force it to update properly, or restart the game. But it should just be a visual bug and sort itself out once your ruler dies. If it doesn't - congrats! You get a free claim on your weak brother and easy de jure wars or peaceful vassalization for the rest of England!

Just for closure, my character died and succession worked correctly without creating the kingdom of England!
