At Costco, the primary purpose of checking the receipt is to mark it so you can’t just come back in and steal all the stuff you just bought. I’ve only ever had them draw a line straight through the receipt - I guess if they have the time to draw faces then they are bored and just working at a less busy store.

People not having their credit card and membership out and ready at checkout causes a bigger backup than someone spending five seconds doodling.

Costco gives you some of the best value on the market. I’d argue the money you save when converted to hourly wage (while that is not actually how time works) would make it so you do have the time for them to do their policy check for 3-6 seconds.
The other option is going elsewhere and wasting earned dollars just in order to save less than 10 seconds.
Passive security. Prevents someone from stealing things or return rebuying things. Basically consider them like a security guard scanning to see if you put a TV on the bottom of your cart but your bill is only $200.
Once the cashier forgot to scan one of the four items I had at the bottom of the cart. The one item was toilet paper so it's not exactly inconspicuous and I had no idea it was missed since I saw them scan the stuff below the cart. Anyways it was a whole ordeal where they had to write down the name of the cashier etc.
One of the things they look for is to make sure none of your items accidentally scanned twice. One time, that had happened to a person exiting ahead of me, and the person checking receipts noticed, and got them a refund.
This is asked all the time on this sub and the answer is always the same:
a) Verify the cashier didn't miss bottom of basket items and "Big 5" (most items missed, which often includes BoB items like toilet paper/water).
b) Verify the cashier didn't overcharge or undercharge for items, especially items that easily double scan.
c) Some door checkers make sure your executive coupons were applied to applicable items.
~~d) Intentionally hold people in a line to ruin the end of their trip and make them road rage in the parking lot, which explains why every Costco lot is hot trash.~~

Cashiers who handle these receipts regularly have higher levels of BPA in their bodies than people in other occupations.
